If it were me, I would give the understudy the role. I am not much of an acting guy anyway and I would probably get too nervous around an audience. Also, not giving the role to them just because of something they did in elementary school is incredibly petty and I don't think a kiss scene between actors is that big a deal on a couples's relationships.
